If you are making a Tribunal referral, you need to download the  Form of Authority and the Data Consent Form in advance, as you will be required to attach a completed copy to this referral. You do not need to complete these if you are referring yourself for help.
Please note we only accept referrals for people who live in London, are below pension age, and are in receipt of, or would be eligible for, a means-tested benefit. Please confirm:
